Here are the instructions to allow you to book an appointment for interview and examination for this report.  Current schedules for patients over 16 years are as follows:

Tuesday lunchtimes 12-2 The Clementine Churchill Hospital, Sudbury Hill, Harrow, HA1 3RX

Wednesday mornings 8 -1 The Platinum Medical Centre, 15-17 Lodge Rd, London NW8 7JA

Wednesday afternoons 2-5 The Platinum Medical Centre, 15-17 Lodge Rd, London NW8 7JA

 

 You can check Mr Gault's availability online here, choose the Medical Report option and the date and time, click to reserve it then telephone us on 0203 393 1103 to let us know so that we can confirm it. Please give the name of the patient/client and let us know that you are attending for a medical report appointment. The appointment usually lasts one hour.

When you attend the appointment please bring along any information, including diaries, notes and photographs, pertaining to the injury you have suffered which you feel might assist Mr Gault in describing your case in his report.
